Iron Dextran (Parenteral)

Trade Names: Infed, Dexferrum
Drug Class: Antianemia agent (iron supplement)
Preparations: Injection, 50 mg/mL (2 mL)
Dose: Test dose 0.5 mL i.v. or i.m. Observe for at least 1 hour before administering dose. Calculate dose by referring to the treatment nomogram in the package insert and administer accordingly. Intravenous injection must be slow. Infuse very slowly initially, and observe for allergy or anaphylaxis. Intramuscular regimen may require several injections on different days (maximum at one site is 2 mL).
Indications: Treatment of iron deficiency anemia in patients unable to take oral medication
Mechanism of Action: Replaces deficient iron stores
Contraindications: Hypersensitivity to iron preparations, hemochromatosis
Precautions: Administer under medical supervision with facilities available to treat anaphylaxis. Administer test dose first. If administered by intramuscular injection, it must be by a deep intramuscular injection, using a Z-track technique, in the upper outer quadrant of the buttock (not in upper arm).
Monitoring: Monitor for allergy/anaphylaxis during administration. Use hemoglobin and reticulocyte count to monitor response.

Adverse Effects
Common: Dizziness, fever, sweating, nausea, vomiting, metallic taste, discolored urine, staining skin at site of injection, leukocytosis
Less common: Anaphylaxis, cardiovascular collapse, urticaria. Onset of arthralgia, sweating, urticaria, and dizziness may be delayed 1–2 days after administration.
